SUMMARY:Navigating Together Support Group
DESCRIPTION:This six-session\, weekly support group is an opportunity for people living with dementia\, or Mild Cognitive Impairment\, and their care partners to join a support group together. Here pairs can come together to share and find support from others navigating similar challenges or concerns. These semi-structured sessions offer a space to explore topics such as coping with changes\, communication\, evolving relationships and roles\, maintaining independence\, and planning. \nThis short-term support group aims to provide a space where participants can share experiences from their perspective as a person living with dementia or as a care partner. Participants are welcomed as pairs to explore ways to adapt to changes together\, find a new normal\, and discuss and share unique strategies that support autonomy\, safety\, and self-care.\nThe group is facilitated by Social Workers at the Alzheimer Society of Toronto who help promote informative and supportive group discussion. \nScreening is a prerequisite for attending this Support Group. \nSession Dates: \nWeek 1 – April 17th\nWeek 2 – April 24th\nWeek 3 – May 1st\nWeek 4 – May 8th\nWeek 5 – May 15th\nWeek 6 – May 22nd \nPlease note\, participants are expected to commit to at least 4 sessions listed above. \nPlease select 2 tickets and enter the required information for both attendees when registering.
