Intake Process
If you are a care partner, family member or a person living with Alzheimer’s disease and other dementias, follow these steps to connect with one of our Intake Coordinators who help connect you to our services and support.
Get connected
Click “Get Started” to fill out our easy and secure online form. You can also ask your physician or health care provider to complete a referral for you.
Speak with someone
After sending the form, you’ll receive an email confirmation and be contacted by one of our Intake Coordinators to get you started on your journey and confidentially discuss your needs and plan your next steps.

Following your call, you will be connected to services most relevant to you, and receive information tailored to your needs.

First LinkⓇ Care Navigator
Navigating the health and social service system can be complicated. We’re here to help.
If you need assistance connecting to dementia support and services, are having trouble coordinating your existing services, or simply don’t know where to start, a First Link Care Navigator can be your go-to person, and
- Help assess your needs

What You Can Expect
An important step in connecting with services at AST is speaking with an Intake Coordinator. Intake Coordinators will talk to you about your dementia specific needs and interests to determine which of our free programs, services, and supports are the most appropriate for you at the time of contact. An Intake Coordinator may refer you directly to one of our programs OR connect you to a First Link Care Navigator or Social Worker to discuss your needs in greater detail and create a support plan that is unique to your needs.
Counselling
Using a goal-oriented approach, our social workers provide safe and confidential counseling with a focus on dementia related challenges and concerns. Conversations with a social worker can include: discussions about your experience with dementia, education on common symptoms and changes, developing coping strategies to better manage your/a family member’s cognitive changes, and future planning.
