What is the Care Partner Mobile App?
The Care Partner Mobile Application is a rebranding and expansion of the Dementia Talk App, an award winning app created by the Reitman Centre at Mount Sinai Hospital, designed to support caregivers (family and friends) to someone living with dementia, ultimately promoting everyone’s well being.
What will I find on the application?
The Care Partner App has tools and resources to support caregivers to someone living with dementia.
The tools include:
- Behaviour Tracker
- Behaviour Care plan
Other Features Include:
- A Scheduler
- A Team builder
- Caregiver Corner with resources that focus on caregiver wellness
- Other external resources
Where can I download the app?
The Care Partner App is available for download on both the Apple and Google Play stores.
