Cantonese Peer Support Group for Care Partners

This Cantonese Peer-led Support Group provide care partners to connect with one another and learn and discuss the practical and emotional needs related to caring for a family member with Alzheimer’s Disease or other dementias. Each session will be broken down into two components: An Educational presentation followed by discussion and sharing related to the presentation topic. Groups are facilitated by volunteers who have lived experience as care partners and have received training on group facilitation. Participants and peer facilitators decide on the focus and direction of the group.
粵語支援小組,旨在為照顧者提供互相聯繫的機會 、並且學習和討論有關照顧患有阿茲海默氏症或其他認知障礙症家庭成員時的實際和情感需求。每節分為兩個部分:教育講座,然後有關主題的討論和分享。小組由有照顧經驗並接受過小組輔導訓練的義工帶領。參加者將會和主持人一起決定小組的重點和方向。
 
 
Learning Objectives:
• Provide a safe setting which promotes mutual support, both practical and emotional, throughout the course of the disease
• Enhance care partner’s knowledge of the illness, available supports, and coping strategies.
小組目的:
• 提供安全的環境,促進在疾病過程中的互助、實用知識交流與情感支援。
• 提高照顧者對疾病,尋找適切的支援,應對策略等知識。
 
 
Support Group will be presented in CANTONESE using Zoom.
The group will meet on: 
Every 2nd Thursday of the month from 4:00 pm to 5:30 pm
小組將以粵語ZOOM會議形式進
每月第2個星期四,下午4時至5時30分
Registration is required followed by a short pre-screening by phone call prior to the start of the program.
參加者須事先登記,進行簡短電話篩選程序。

Bereavement Support Group (In Collaboration with Hazel Burns Hospice) – Zoom

This 8-week online support group meets to provide participants with an opportunity to support each other through the grief and bereavement process. Both practical and emotional needs will be explored to help participants cope with a loss.
Learning Objectives:
– Opportunity to share insights and experiences with each other related to their personal caregiving experiences, as well as grief and bereavement
– Learn from others on adjusting to the loss and their own changing relationship to self and others.
– Discuss and share different strategies regarding selfcare including discussion on stages of grief (various frameworks), finding meaning and exploring feelings related to loss and loneliness.
– Receive updated information on where to seek additional supports if desired


Session dates:
Wednesdays from 6:30pm – 8:30pm, starting Sept 4th ending Oct 23rd.
– Week 1 – Wed Sept 4th
– Week 2 – Wed Sept 11th
– Week 3 – Wed Sept 18th
– Week 4 – Wed Sept 25th
– Week 5 – Wed Oct 2nd
– Week 6 – Wed Oct 9th
– Week 7 – Wed Oct 16th
– Week 8 – Wed Oct 23rd


* Please note, participants are expected to commit to all 8 sessions listed above. Kindly register only if these dates work for you. Your contact information will be shared with Hazel Burns Hospice who will contact you regarding screening. Please check your junk/spam if you do not receive a confirmation email upon registration. Zoom information will be provided via email approximately 24-48 hours prior to the first session. This program is not a crisis support. The Community Connections Cafe – 20 Eglinton Avenue West

The Community Connections Café is a welcoming space where people with memory loss or Dementia, and their care partners, can come to connect with others and support their brain health while socializing. This is an opportunity to enjoy simple refreshments and make new friends in a Dementia Friendly Community Space
 
The Community Connections Cafe focuses on social wellness for people with memory loss or Dementia, and their care partners. The program is informal with the intended atmosphere of a local coffee shop where participants can chat over a beverage, do something creative, or play a game.
 
 
Every 3rd Wednesday of the month from 2:30pm to 4:30pm In-person at our AST Head Office.
 
Location: 20 Eglinton Ave West, 16th Floor, Toronto
(Northwest corner of Yonge and Eglinton, across the street from Yonge and Eglinton subway station. Our office tower is to the left of Metro Grocery)

 

 
Registration is required for this program and closes at midnight (12am) the day prior to the event.
Please specify the number of guests attending when you register. You may register up to 3 persons per group.

Sexuality, Intimacy, and Dementia (For Care Partners) – ALZeducate

This workshop identifies and differentiates intimacy and sexuality, highlights changes for the person living with dementia and their partner, supports participants to understand sexual behaviours in dementia and offers strategies to help manage sexual behaviours.
